UNR Security Application Web Form
You must submit a Security Application form to gain access to secured UNR systems such as:
- Advantage / Financial Data Warehouse
- HRMS - CAIS HR (Is now a separate form - click here)
- Peoplesoft / MyNevada
- Nolij Web
- ONE-LOOKUP Application and Reports
- 3rd party applications such as Astra and Continuity 2000
The main Security Application form must be completed regardless of the specific system to which you are requesting access. For each system, an additional
application form must be completed. For your convenience, these applications have been combined into one Security Application web form.
The security application must be completed online, but cannot be submitted online. Paper copies with signatures are still required. It is advisable that you
read the procedures for each system and gather all necessary information before you begin to fill out the form. The information that you enter will not be saved
when you exit your browser.
When you open the Security Application web form, you will initially see only the Application for Administrative Account form. When you check a box (or boxes)
under the "Applications and Services Requested" section, the appropriate additional form(s) will be added to the web form. When you click
the "Print" button at the top of the webform, each security form request will print on a separate sheet of paper. You and the person authorizing
your request must sign EACH FORM.
Please note: If your name and department information does not print onto each application main page,
the application will be returned to you to be printed correctly.
